Manuel Locatelli pulled one back, Dejan Kulusevski notched the equaliser and then Mattia De Sciglio completed the dramatic turnaround in the 77th minute.
Juventus were reduced to ten men after Matthijs de Ligt was sent off for handling the ball in the area and a penalty was awarded to the hosts.
Pellegrini stepped up but unlike his previous set-piece effort, the Italian's spot-kick was tame and Wojciech Szczęsny saved before the rebound was put wide.
