The Citizens secured hard fought win over Marco Silva men at the Etihad Stadium on Saturday evening.
Erling Haaland came off the bench to score the winner in stoppage time after Andreas Pereira scores equaliser to cancel Alvarez opener.
Manchester City were in control of the game when the Argentine forward put them in the lead.
But Cancelo was shown straight red after he shoulder-barge Harry Wilson in the area.
Fulham made it 1-1 after former Manchester United midfielder Andreas Pereira scored from the penalty spot.
Manchester City played 65 minutes with 10 men but got the three points after Haaland converted his penalty in the 95th minute.
Speaking after the game, Pep Guardiola conceded that Joao Cancelo deserved to be sent off in Manchester City's win over Fulham.
"It's the last man, soft contact or not soft contact, there cannot be contact," Guardiola said. "It's a deserved red card. I said many times to players, when you have 85 minutes and are out of the 18-yard box, make a foul. You have 65 minutes left, score a goal, 1-1, okay we have time with 11 v 11. Instead, it's penalty, 1-1 and 65 minutes with 10 men. Hopefully we can learn."
